Abstrak

The implementation of work safety is one of the important requirements in a company, such as PT. Surya Karya Setiabudi (SKS). As a company engaged in construction, there needs to be special attention to every employee regarding occupational safety and health which has a very high risk and causes many losses to the company and employees. Problems that still often occur are work accidents that occur such as, slipping, electric shock, falling from height, welding fire, being splashed with cement. The purpose of the research is to determine the level of danger and risk of K3 in corrective and maintenance actions as well as to prevent and control work accidents. This study uses the method of Hazard Indentificatian, Risk Assessment Control (HIRARC) to identify, assess, and control risks that occur and Failur Mode And Effec Analysis (FMEA) to examine work accidents at PT. Surya Karya Setiabudi, and analyze the failure of a system or equipment and evaluate the effects of the failure. The results of hazard identification using the HIRARC method obtained risk values with risk level categories, including: 3 risk levels  with Low category, 4 risks with High category, 3 risks with Medium category, and 8 risks with  existing Extreme categories   . While the results of research using the FMEA method, that the highest RPN (Risk Priority Number) value identified by the source of risk there are several high values contained in several risks, including: falling from a height of 240, being hit by heavy equipment with a value of 243, road accidents with a value of 320, and falling from a height with a value of 360.
